Once your appointment is scheduled, you will receive get-to-know you forms (aka intake paperwork) via ChARM patient portal, which is a secured connection. An invitation is sent to your email address and then you create an account where you can access documents, forms, labs, treatment plans, and more.
Forms are best completed at least 1-2 weeks prior to your appointment. That way, the review process is thorough and your health can be given the respect and time it deserves.
What is telemedicine? Telemedicine is a flexible way to have appointments virtually either by telephone or video conference (through a secure platform). Because I'm licensed in both Oregon and Washington, this is a way for me to serve more patients.
Initial visits are typically an hour and a half. An extensive review of your intake paperwork is done prior to your visit, which is one of the reasons why it's so important to complete your intake forms 1-2 weeks prior to your appointment. In the visit, we will discuss your goals, expectations, and I will ask questions to try and better understand how we can best work together.
Follow-up appointments are generally 30-45 minutes. When first establishing care with patients, I typically like to work with patients at least once a month for three to six months. This is highly dependent upon your unique circumstances and health goals.
I do not accept insurance at this time. I am more than happy to provide you with "superbill" (an itemized list of services) that you can submit to your insurance company.
Payment is due at the time of service. Accepted payments include: credit card, check, Paypal, Venmo, or Square.
